CCUC Tax Exempt Form
As a church, purchases for ministry related items are exempt from sales taxes. You may present this form at retailers to ensure that your purchases are not assessed sales taxes. YOU MAY NOT USE THIS FORM FOR ANY PERSONAL PURCHASES.

CCUC-North Youth Handbook
